The Washington Crime Victims Compensation Program Initial Response and Assessment: Form II is an application form used to determine if a victim of a crime is eligible for financial assistance through the state's crime victims compensation program. This form is the first step in determining whether the victim can receive compensation for medical and/or mental health expenses, lost wages, or other crime-related expenses. The Form II also collects information on the applicant's family dynamics, prior criminal history, and other relevant information. There are three types of Washington Crime Victims Compensation Program Initial Response and Assessment: Form II: 1) Form II-A for victims of violent crime; 2) Form II-B for victims of economic crimes; and 3) Form II-C for victims of stalking and/or domestic violence.
