Washington Hearing Aid Repair/Durable Medical Equipment Provider Hotline Service Authorization Request is a service provided by the Washington state government that allows individuals to submit requests for authorization of hearing aid repairs or durable medical device repairs. This service is available to both individuals and providers. Individuals can submit requests for authorization of repairs for their hearing aid or durable medical device, while providers can use the service to request authorization for repairs on behalf of their clients. There are two types of Washington Hearing Aid Repair/Durable Medical Equipment Provider Hotline Service Authorization Requests: one for repairs to hearing aids and one for repairs to durable medical devices. The request requires information such as the client's name, address, and phone number; the provider's name and contact information; the manufacturer, model, and serial number of the device; and a description of the problem. The request must also be accompanied by a copy of the client's insurance card and the provider's NPI number. Upon receipt of the request, an authorization will be issued if the repair is approved.
