Washington Instructor's Report of Accident / Incident is used to document accidents or incidents that occur during instruction for a student who holds a pilot certificate or a student enrolled in a training course. It is used to track and analyze trends in instruction and to provide feedback to instructors. The report includes details about the incident or accident, the student's performance, and any follow-up action taken. There are two different types of Washington Instructor's Report of Accident / Incident: 1. Preflight Accidents/Incidents: This report includes information about accidents or incidents that happen before the aircraft is in the air, such as ground accidents or incidents that occur during pre-flight inspections. 2. In-Flight Accidents/Incidents: This report includes information about accidents or incidents that occur while the aircraft is in the air, such as engine failures, in-flight structural problems, or pilot errors. It also includes information about any evasive maneuvers taken to avoid hazards.
