The Washington Manufactured Home Installer Certification Tag Transfer Request form is used to transfer the ownership of an installed manufactured home installer certification tag. This form can be used by the original installer, the new owner of the tag, or the new owner’s authorized representative. The form is divided into two parts: the Tag Transfer Request portion and the Applicant’s Signature Authorization portion. In the Tag Transfer Request portion, the original installer must provide their name, address, and license number, as well as the name, address, and license number of the new owner or their authorized representative. In the Applicant’s Signature Authorization portion, the new owner or their representative must provide their name, address, and signature.
