Washington Manufactured Home Installer's Monthly Certification Tag Report is a document used to certify that a manufactured home has been installed in accordance with Washington State laws and regulations. It is issued to the installer of the home and is valid for one month. The report includes the manufacturer's name, model number, and serial number; the installer's name and license number; the date of installation; the location of the manufactured home; and the certification tag number. There are two types of Washington Manufactured Home Installer's Monthly Certification Tag Report: Site-Built Installation Report and Prefabricated Installation Report. The Site-Built Installation Report is for homes built on-site, while the Prefabricated Installation Report is for homes that have been delivered to the site in one or more sections. Both reports must be completed and submitted to the Department of Licensing for review and approval.
