There are two types of Washington Network Provider Application Packets: one for individual providers and one for provider organizations. The individual provider packet includes a general information form, a professional information form, a service information form, a financial information form, and a criminal background check authorization form. The provider organization packet includes the same forms as the individual provider packet, but also includes a provider organization information form, a provider organization authorization form, and a provider organization financial statement. Providers must submit all forms and documents in the Washington Network Provider Application Packet in order to be considered for inclusion in the network.
