Washington Request for Duplicate Elevator Certificate is an official document required by the Washington State Department of Labor & Industries (L&I). It is used to replace an existing elevator certificate that is lost, damaged, or otherwise unavailable. This document is required for any new or existing elevator or escalator operating in the state of Washington. It is issued to the elevator owner or the authorized agent of the owner. There are two types of Washington Request for Duplicate Elevator Certificates, the General Certificate and the Limited Certificate. The General Certificate is issued when a new or existing elevator has been inspected and approved by an L&I inspector and meets all applicable requirements of the Washington State Elevator Safety Act. The Limited Certificate is issued when an elevator is only being used for private use or in a research or experimental setting and does not require full inspection and approval. Both types of certificates require a fee and must be accompanied by an Elevator Certificate Application and applicable supporting documentation. Upon approval, the duplicate certificate will be issued by L&I.
