The Washington Victim Verification Form is a document used by victims of crime in the state of Washington to verify their identity and the facts surrounding the crime. It is used to help law enforcement and prosecutors in the investigation and prosecution of criminal cases. There are two types of Washington Victim Verification Forms: the Washington Victim Verification Form (VLF) and the Washington Victim Verification Form — Enhanced (VVF-E). The VVF is used by victims to provide identifying information such as name, address, date of birth, Social Security number, and other personal identifying information. The LIFE is used in cases of serious violent crimes and includes additional information such as relationship to the offender, crime details, and number of times the crime was committed. Both forms are available online and can be completed electronically or printed and filled out by hand.
