Washington WISH 10 for Agriculture Workers — Class Host Application is a form that must be filled out by employers or volunteer organizations hosting a WISH 10 for Agriculture Workers course. This application is used to register the course and provide the Washington State Department of Labor & Industries (L&I) with the necessary information to properly certify the class. The application must be completed and submitted to the L&I at least 30 days before the start of the class. The application consists of three parts: Course Information, Host Information, and Instructor Information. Under Course Information, the employer/volunteer organization must provide information such as the start and end dates of the course, the total number of hours, the maximum number of students, and the location of the class. Under Host Information, the employer/volunteer organization must provide contact information for the host as well as the name of the person responsible for the course.
