A Wisconsin Notice of Objection to Claim is a document filed by a creditor with the Wisconsin Department of Revenue (FOR) in order to object to a claim made by a debtor. This document informs the FOR of the creditor's disagreement with the debtor's claim for a tax refund, credit, or other payment. If the FOR agrees with the creditor, the claim will be denied. There are two types of Wisconsin Notice of Objection to Claim. The first type is a Notice of Objection to Claim of Refund, which is used to object to a claim for a refund of taxes. The second type is a Notice of Objection to Claim of Credit, which is used to object to a claim for a credit against taxes. The Wisconsin Notice of Objection to Claim must include the creditor's name, address, and contact information; the debtor's name and address; the taxpayer identification number; a description of the claim being objected to; the reason for the objection; and any supporting documentation. The notice must be signed by the creditor and mailed or faxed to the FOR. Once the notice is received, the FOR will review the objection and make a decision.
