The Wisconsin Examining Physicians Or Psychologists Report is a document used by the Wisconsin Department of Safety and Professional Services (DSP) to evaluate the eligibility of an applicant or licensee to practice medicine or psychology in the state. This report is used to assess the individual's professional qualifications, educational background, training, experience, physical and mental health, and any relevant criminal history. The report includes a written evaluation of the applicant's qualifications and a statement of the DSP Board's action. There are two types of Wisconsin Examining Physicians Or Psychologists Reports: the Initial Report and the Renewal Report.
