The Wisconsin FA-604B Order to Change: Custody/ Placement/Support/Maintenance/Arrears is a form used by the courts in Wisconsin to modify an existing order or to create a new order in regard to custody, placement, support, maintenance, and/or arrears in family law cases. This form is used by the court to modify the existing orders in cases involving children or to make a new order regarding the support, placement, and/or custody of a child. There are three different types of Wisconsin FA-604B Order to Change: Custody/ Placement/Support/Maintenance/Arrears: 1. Custody/Placement — This type of order is used to modify the existing order in regard to legal custody, physical placement, and/or visitation rights for the child. 2. Support/Maintenance — This type of order is used to modify the existing order in regard to support and/or maintenance payments for the child. 3. Arrears — This type of order is used to modify the existing order in regard to arrears payments owed for past due child support.
