The Wisconsin Affidavit of Service (Probate) is a court document used to certify that certain documents have been served to a party or parties involved in a probate action. It is a sworn statement made by the individual who served the documents that the service was done in accordance with the laws of Wisconsin. The affidavit must include the name of the party or parties served, the manner of service, the date of service, and any other information as required by the court. There are three types of Wisconsin Affidavit of Service (Probate): Personal Service, Substituted Service, and Constructive Service. Personal Service requires that the documents are delivered to the party or parties personally. Substituted Service requires that the documents are delivered to a person of suitable age and discretion at the place of residence of the party or parties. Constructive Service requires that the documents are mailed to the party or parties by registered mail.
