Wisconsin Application for Ancillary Administration (WAFT) is an online form issued by the Wisconsin Department of Safety and Professional Services. It is a mandatory application for those individuals who wish to obtain an ancillary administration license in Wisconsin. WAFT is used to collect information about the applicant, including their education, experience, and qualifications. The application also includes questions related to the applicant’s criminal history. There are two types of WAFT applications: one for an Ancillary Administrator and one for an Ancillary Administrator Intern. Ancillary Administrators are responsible for managing the day-to-day operations of, or providing support services to, a licensed long-term care facility or other health care facility in Wisconsin. Ancillary Administrator Interns are those individuals who are in the process of obtaining their Ancillary Administrator license.
