Wisconsin New Part 1A and 1B of Form ADV are both registration forms filed by investment advisors with the Wisconsin Department of Financial Institutions (DFI). Part 1A is the initial registration from the investment advisor must file with the DFI when registering to do business in Wisconsin. It requires the investment advisor to provide basic information about the advisory firm, including its name, address, contact information, and information on any affiliated persons. Part 1B is the annual amendment to Part 1A. It requires the investment advisor to update information provided in the initial registration, including changes in its personnel, services, and fees. It also requires the investment advisor to disclose information about any disciplinary events that have occurred since the initial filing. There are three types of Wisconsin New Part 1A of Form ADV: • Part 1A-N for newly registered investment advisors; • Part 1A-A for amendment filings; and • Part 1A-S for revoked investment advisors. Wisconsin New Part 1B of Form ADV also has three types: • Part 1B-N for newly registered investment advisors; • Part 1B-A for amendment filings; and • Part 1B-S for revoked investment advisors.
