Wisconsin Unincorporated Nonprofit Association — Appointment of Agent is a legal document used to appoint an agent to accept service of process on behalf of the unincorporated nonprofit association. The appointment must be filed with the Wisconsin Department of Financial Institutions in order to be effective. The agent must be an individual Wisconsin resident, a Wisconsin corporation, or a Wisconsin limited liability company. There are two types of Wisconsin Unincorporated Nonprofit Association — Appointment of Agent: Initial Appointment of Agent and Amendment to Appointment of Agent. The Initial Appointment of Agent is used to designate the initial agent for service of process, while the Amendment to Appointment of Agent is used to change the designated agent or to update the address of the agent. Both types of documents must be signed by a representative of the unincorporated nonprofit association.
