The Wisconsin Four Year Notary Commission Application is an official form used by individuals in the state of Wisconsin to apply for a four-year commission as a notary public. The application must be completed in full and submitted to the Wisconsin Department of Financial Institutions. There are three different types of Wisconsin Four Year Notary Commission Application: Original, Renewal, and Reinstatement. The Original Application is used by individuals who have never held a notary commission in Wisconsin before. The Renewal Application is used by individuals who are renewing their existing notary commission. The Reinstatement Application is used by individuals who have previously held a Wisconsin notary commission, but it has expired or been revoked. All applications must be completed in full and signed in front of a notary public. Applicants must also include a completed background check and a $10 fee. Once the application has been reviewed and approved by the Wisconsin Department of Financial Institutions, the applicant will be issued a four-year notary commission.
