The Wisconsin Information Statement (WIS) is a document containing detailed information about a company's business activities in the state. It is required for certain business entities that are registered with the Wisconsin Department of Financial Institutions (DFI). The WIS must be completed annually and is available in both electronic and paper forms. The WIS includes information about the company's business activities, including the type of business, the number of employees, the total amount of assets, revenues, and expenses, the names and addresses of the owners, directors, and officers of the company, and the company's financial condition. There are two types of Wisconsin Information Statement: the Annual Statement and the Change of Information Statement. The Annual Statement is an annual filing that is due by April 15th of each year and is used to report a company's financial activities. The Change of Information Statement is used when there has been a change in the company's information, such as a change in the name of the business, the address of the business, the ownership structure, or the number of employees.
