Wisconsin Notice to Ward of Request to Transfer Guardianship to WI is a document used to transfer guardianship of a ward from one state to Wisconsin. The document must be signed by the current guardian and the court where the current guardianship has been granted. The Notice informs the ward that the guardianship is being transferred to Wisconsin and includes the name of the new guardian and the name of the court that will be granting the new guardianship. The Notice also informs the ward that they may object to the transfer and provides information on how to do so. There are two types of Wisconsin Notice to Ward of Request to Transfer Guardianship to WI: Out-of-State Guardianship Transfer Notice and In-State Guardianship Transfer Notice. The Out-of-State Guardianship Transfer Notice is used when the ward's guardian is located outside of Wisconsin, and the In-State Guardianship Transfer Notice is used when the ward's guardian is located in Wisconsin.
