The Wisconsin Report of Guardian ad Item (GAL) is a legal document used by the court to provide a recommendation to the court regarding the best interests of a minor or incapacitated person. It is typically prepared by a Guardian ad Item — a court-appointed representative – who is assigned to investigate and make recommendations about the minor’s or incapacitated person’s situation. The GAL will review all available evidence related to the case and make a recommendation to the court based on their findings. The Wisconsin Report of Guardian ad Item includes details of the GAL’s investigation, the GAL’s recommendation, and a summary of the information reviewed. There are three types of Wisconsin Report of Guardian ad Item: the Request for Appointment, the Report to the Court, and the Final Report. The Request for Appointment is the initial document filed when a GAL is appointed to a case. The Report to the Court is filed before a hearing to provide the court with an update on the GAL’s investigation. The Final Report is filed at the end of the case and provides the court with the GAL’s final recommendation.
