The Wisconsin Affidavit of Service of SC Summons-Complaint Corp Or LLC is a document that is signed by an individual (the “Server”) who has served legal documents (the “Summons and Complaint”) to a corporation or limited liability company (the “Defendant”) in accordance with the Wisconsin Rules of Civil Procedure. The Server must sign the Affidavit of Service and present it to the court in order to prove that the Summons and Complaint were properly served. This document must include the date of service, the type of service (in-hand, first-class mail, express mail, or another method of service), the Defendant’s name and address, and a description of the documents served. There are two types of Wisconsin Affidavit of Service of SC Summons-Complaint Corp Or LLC: one for service of process to a corporation, and one for service of process to a limited liability company. The two forms are nearly identical, except that the form for service of process to a corporation requires additional information about the corporation, such as its date of incorporation and the name of its registered agent.
