The West Virginia Chapter 11 Affidavit for Entry of Discharge Order Prior to Completion of Plan Payments is a legal document required by the U.S. Bankruptcy Code when filing a bankruptcy case in the state of West Virginia. It is used to provide evidence that the debtor has made all payments due under the debtor’s Chapter 11 repayment plan and is ready to receive an entry of discharge. The affidavit must be signed by the debtor, and a copy must be filed with the court.
