West Virginia Initial Notice Under WV Code Sec. 23-4-10(f) to Recipients of 104-Week Award Paid In Monthly Payments is a notification that must be provided to recipients of 104-week awards paid in monthly payments. This notification is required under the West Virginia Code Section 23-4-10(f). There are two types of West Virginia Initial Notice Under WV Code Sec. 23-4-10(f) to Recipients of 104-Week Award Paid In Monthly Payments: one for recipients who are receiving payments through direct deposit and one for recipients who are receiving payments by paper check. For recipients receiving payments through direct deposit, the Initial Notice must include the following information: the recipient's name, address, Social Security number, and bank account information; a statement that the recipient has the right to receive a monthly payment in lieu of the 104-week award paid in a lump sum; the amount of the monthly payment; the date the payment will be deposited; and a statement that the award is subject to offset by certain agencies. For recipients receiving payments by paper check, the Initial Notice must include the following information: the recipient's name, address, Social Security number, and bank account information; a statement that the recipient has the right to receive a monthly payment in lieu of the 104-week award paid in a lump sum; the amount of the monthly payment, the date the payment will be issued; and a statement that the award is subject to offset by certain agencies. The West Virginia Initial Notice Under WV Code Sec. 23-4-10(f) to Recipients of 104-Week Award Paid In Monthly Payments must also include a statement that the recipient may appeal the decision to award the 104-week award in monthly payments and a notice of the availability of a telephone number for questions and assistance.
