Wyoming CAMP 1. List of Forms is a set of documents that must be completed by employers, workers, and their representatives in order to comply with the Wyoming Construction Safety and Health Program (CSP). This set of forms consists of a series of documents that must be completed in order to ensure the safety of workers on construction sites. The forms are divided into two parts: Part 1 covers general safety requirements and Part 2 covers specific requirements for each type of construction work. There are four different types of forms: 1. The Employee Notification Form is used to inform employees of their rights and responsibilities under the Wyoming CSP. 2. The Site Safety Plan Form is used to document the safety plan for a construction site and includes information such as hazard identification, hazard control measures, emergency procedures, and safety training. 3. The Hazard Communication Plan Form is used to document the communication plan for a construction site and includes information such as the communication process, hazardous materials' identification, and hazardous materials control. 4. The Hazard Recognition Form is used to document the recognition and evaluation of potential hazards on a construction site. This form includes information such as the identification of the hazard, the evaluation of the potential risks associated with the hazard, and the control measures that should be taken to mitigate the risk.
