Wyoming CAMP 10. Affidavit of Imputed Income is a document that is filed by an employer with the Wyoming Department of Workforce Services when there is a dispute over the amount of wages and/or benefits that an employee should receive. This document is used to calculate the amount of wages and/or benefits that the employee should receive based on the employer’s stated reason for not paying them the full amount. The Affidavit includes the employer’s name, address, state identification number, and the employee’s name, address, and Social Security number. The employer must also state the reason for the dispute and provide evidence to support their claim. There are two types of Wyoming CAMP 10. Affidavit of Imputed Income: regular and limited. The regular version is used when the employer has provided a full explanation of why they are not paying the employee the full amount of wages and/or benefits, while the limited version is used when the employer has only provided a brief explanation for the dispute. Both versions require the employer to provide evidence to support their claim.
