Wyoming MCP 4. Petition for Appointment of Guardian of a Minor is a legal document used to petition the court for the appointment of a guardian for a minor. This document should be completed by the petitioner and filed with the court. There are two types of Wyoming MCP 4. Petition for Appointment of Guardian of a Minor: (1) an initial petition for appointment of guardian of a minor and (2) a petition for the appointment of a new guardian of the minor. The initial petition must include the minor's name, the petitioner's name and relationship to the minor, the facts concerning the minor's need for a guardian, and a proposed plan for the guardianship. The petition for a new guardian must include information about the current guardian, the facts concerning why the new guardian is being proposed, and a proposed plan for the guardianship. The petitioner must also provide evidence of their qualifications and eligibility to act as a guardian.
