The Wyoming Form of Case Management Plan (updated 05/29/13) is a form used by the Wyoming Department of Family Services (DFS) to provide an overview of the services and interventions a child or family will receive in order to support a safe and successful transition to permanency. The form includes a variety of components, including: • Person-CenterePLAla— – The Person-Centered Plan outlines the specific services, activities, and interventions the child or family will receive to meet their individual goals and needs as identified in the Child and Family Team Meetings. • Family Support ServicePLAla— – This plan outlines the services and supports the family will receive from the DFS, such as parenting classes, budgeting, and home visits. Educationalla— – This plan outlines the educational services the child will receive, such as tutoring, GED classes, and referrals to special education services. • HealtCarpalla— – This plan outlines the health care services the child will receive, such as medical care, mental health services, and referrals to specialists. • PermanencPLAla— – This plan outlines the steps the DFS and the family will take to transition the child to permanency, such as kinship care, adoption, or guardianship. There are two different types of Wyoming Form of Case Management Plan (updated 05/29/13): the Initial Plan and the Quarterly Plan. The Initial Plan is used in the first 90 days of case management to outline the services and interventions the child and family will receive.
