ADS-B is required in Class A, B and C airspace within U.S. domestic airspace and all land mass regions of the U.S. as defined in 14 CFR 1.1 and it includes the states (contiguous and non-contiguous), U.S. possessions, or territories.

Pilots/operators using aircraft that do not meet ADS-B Out equipage or performance requirements may request an authorization to deviate from the rule to access ADS-B Out rule airspace. The FAA has developed the ADS-B Deviation Authorization Preflight Tool (ADAPT) to manage these authorization requests.

Pilots need a Mode C transponder and a functional ADS-B Out device to fly in Class Bravo airspace. You also need Mode C and ADS-B Out capabilities if you're flying above the Class B ceiling or within its lateral boundaries up to 10,000 feet MSL.

Aircraft not complying with the requirements may be denied access to this airspace. As described in 14 CFR 91.225, ADS-B Out performance is required to operate in: Class A, B, and C airspace. Above the ceiling and within the lateral boundaries of a Class B or Class C airspace area upward to 10,000 feet MSL .

You must be equipped with ADS-B Out to fly in most controlled airspace. Any airspace that requires the use of a Transponder, described in 14 CFR 91.215, also requires aircraft to be equipped with a Version 2 ADS-B Out system.

I don't have ADS-B Out, can I fly in ADS-B rule airspace? Operators of aircraft not equipped with ADS-B Out, or with inoperative ADS-B Out, who wish to operate in ADS-B rule airspace must obtain an ATC authorization before flying.

Without ADS-B Out, you can fly in any airspace except the ADS-B rule airspace defined by FAR 91.225 (see above). Note that ADS-B is not required in Class D airspace, or under a Class B or Class C airspace shelf, unless it lies within a Mode C veil.
