An advance directive encompasses all legal orders which concern your wishes surrounding future medical care. The document will come into play in the case of severe medical situations in which you're not able to communicate your wishes or make decisions. These may include such conditions as a coma, stroke or dementia.

Advance directives generally fall into three categories: living will, power of attorney and health care proxy.

To be valid, an advance directive must be signed by you in the presence of two witnesses, who will also sign. The person you name as a health care agent may not also be a witness. Some states also require the advance directive to be notarized.

An advance directive isn't the same as a DNR DNRs, like POLST forms, require a physician's sign-off and are medical orders, instructing emergency personnel to withhold resuscitation efforts. If you don't want to receive resuscitation, including that information in an advance care directive is not enough.

An advance directive, alone, may not be sufficient to stop all forms of life-saving treatment. Advance directives are legally valid documents that allow you to express your wishes about the kind of medical treatments you want, and instruct a person of your choosing the ability to make health care decisions on your behalf before a situation arises where you are unable to do so.

A prehospital DNR order is generally a simple, one-page document; you don't need a lawyer to prepare it. You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order.
