Emails, even to formal acquaintances, are simply more casual than a paper letter. You may write the title followed by the person's full name or by only their last name. If this is not the first time you are writing to this person, it would be most natural to use only their last name.

These can be titles prefixing a person's name, e.g.: Mr, Mrs, Miss, Ms, Mx, Sir, Dr, Cllr, Lady or Lord, or titles or positions that can appear as a form of address without the person's name, as in Mr President, General, Captain, Father, Doctor or Earl.

If you're mentioning someone by name and title, they would both remain in uppercase letters for the first letter of each. When you're simply describing their job title rather than addressing them directly by the job title, you can use lowercase. For example, you could say, "Dr. Adams is a great physician."
