The passport is a federally issued identification document so be sure to use your full legal name. Middle names can be tricky when filling out your passport application, but don't let that be the one factor that holds you back. A middle initial is acceptable on your passport instead of providing the full middle name.

It is correct to have your initials after the name. If for example someone is Sita R. (say Sita Ramanathan) the passport will be issued with that full name. The old system, especially in South India was naming as R.

When a professional has earned more than one set of post-nominal letters, it is appropriate to display each set of letters after his or her name. This is done in descending order, with the most prestigious letters being first (closest to the name), followed by a comma, then the next set of letters and so on.

Your surname is your family name. It's also called your ?last name.? When filling out applications, type your surname as it appears on your passport, travel or identity document. Do not use initials.

If you use a surname you must furnish the same here. No initials should be written and all initials (if any) in the applicant's name should be expanded. For instance, for the name used above, writing the Given Name as ?P.K.JAIN? or ?PIYUSH K JAIN? is not correct.

The initials of name are meant to be written in the front. You can ask the concern department of school to correct it.

But what if you don't want to use your middle name? Unfortunately, if your middle name appears in your proof of citizenship or identification, you should include it. Luckily, you don't need to include it in full. Instead, you can use an initial in its place.

Will it be a problem if my passport only has my first name and middle name and not my last name? Technically, your passport is invalid. Your passport must match your birth/legal name, to be valid. This can be your birth name on your birth certificate or it can be your legal name from a legal name change document.

Capitalize and spell out formal titles such as president, professor, dean, chairman, etc., when they precede a name (e.g.,Professor Smith). It is preferred that these titles appear in lowercase when the title follows a name or stands alone (e.g., Bob Smith is a professor at Saint Peter's University.).

The correct format to be used in all Government issued documents are First name(Given name) Middle name followed by Last name (family name or father's name). Always expand your initials and mention your full name in all the documents else you will end up with different names on different documents.
