If you had a suspended or restricted driver license and the suspension or restriction period is over, go to your local Secretary of State branch office with sufficient identification and your Social Security number to apply for your driver's license. You may owe a reinstatement fee depending on the type of suspension.

Check Driver's License Status. To obtain your driver's license record, fill out the Michigan driving record request form found on the Secretary of State website. This form can be mailed to the Department of State, or purchased at any branch office for same day service.

The notice explains the reason for the suspension and what you can do if you disagree with it. If you did not get a notice in the mail, you can call the Ohio BMV at 614.752. 7600 or 844.644. 6268 and request that they resend any notices of suspension that were mailed to you.

If you are convicted for failure to reinstate your license, you will face misdemeanor charges. A first and second offense will be considered an unclassified misdemeanor offense. This is punishable by up to 500 hours of community service and a fine of up to $1,000.

License restrictions explain your driving limitations. License restriction codes are printed on the front of your license on the line under the "Expiration date" and "E" Endorsement Fields, after the letter "R". A description of any restrictions you have is printed on the back of your photo license document.

If the Indiana Bureau of Motor Vehicles (BMV) has notified you that you must pay re- instatement fees in order to obtain a valid Indiana Driver's License then you may be eligible to seek a Court Order directing the BMV to waive part or all of those re-instatement fees.
