SECTION III - APPOINTEE ACKNOWLEDGEMENT I understand that I am strictly liable to the United States for all public funds or payment certification, as appropriate, under my control. Certification of Vouchers. DD Form 577 (Appointment/Termination Record ? Authorized Signature) shall be used to appoint and terminate all certifying officers. The DO shall verify all Certifying officers who submit vouchers for disbursements, and shall have access to the DD Form 577 records.

Every competent adult shall have the right to appoint a health care agent by executing a health care proxy. The provision goes on to permit the appointment of an alternate agent, but does not speak of appointing multiple agents at the same time.

Two witnesses must watch you sign your Health Care Proxy form and say that you appeared to sign willingly. Neither your agent nor your alternate agent can serve as a witness. You do not need to have the form notarized.
