Copy number polymorphisms can affect gene expression through complex mechanisms that extend beyond simple gene dosage effects, and include, for example, insertion and deletion of gene regulatory regions and alterations of physical proximity of genes and regulatory elements.

Copy number variations (CNVs) are genomic alterations that result in an abnormal number of copies of one or more genes. Structural genomic rearrangements such as duplications, deletions, translocations, and inversions can cause CNVs.

Like SNPs, smaller CNVs will affect only single genes and thus contribute, together with SNPs, to single-gene disorders. However, unlike SNPs, larger CNVs can affect 2 or more contiguous genes and thus contribute to syndromic or complex disorders caused by defects in multiple genes.

To determine copy number, a relative quantitation analysis is performed between the unknown sample and a calibrator sample (one in which the copy number of the target gene is known). The sample is combined with both assays, master mix, and then run in quadruplicate in the real-time PCR instrument.

A variety of technologies are available to detect CNVs such as fluorescence in situ hybridization (FISH), Array-comparative genomic hybridization (aCGH) (see Unit 4.14), genome-wide single nucleotide polymorphism (SNP) arrays (see also Unit 8.13), and most recently, high-throughput sequencing.
