Extend Name Diploma For Free

You Can Change Your Name at Any Time While You're a Student If you'd like to change your name while studying at university and want your name to appear on your official record, you have to submit a formal request in writing.

You should use whichever name you would prefer to see on the diploma when you look at it. If you like your middle name, then by all means include your full middle name. If you don't like your middle name, then use your middle initial.

Your name will appear on your diploma exactly how you specify on your graduation application. If you have a preferred first name that is not your legal first name, please be sure to put your preferred first name on your graduation application.

To appear on the diploma, a legal name change must be submitted to the Registrar's Office, 1113 Murphy Hall, by the last day of the degree-expected term. Submit a Legal Name Change or Correction form. Documentation supporting the name change is required. There is no fee for this service.

Inform the registrar's office that you have changed your name and require a new issue of your diploma. You will either be required to fax in all the documents or bring them in person. If you are bringing the documents in person, ask for a specific time when you should come in and a contact person to meet with.
