Attestation is the act of witnessing the signing of a formal document and then also signing it to verify that it was properly signed by those bound by its contents. Attestation is a legal acknowledgment of the authenticity of a document and a verification that proper processes were followed.

All passengers travelling to the USA are required by the US authorities to carry a completed and printed version of the "Combined Passenger Disclosure and Attestation Form".

TO THE UNITED STATES OF AMERICA Under United States federal law, the applicable portion of the attestation must be completed for each passenger age two or older and the attestation must be provided to the airline or aircraft operator prior to boarding a flight to the United States from a foreign country.

This Amended Order went into effect for flights departing to the US from a foreign country at or after am EST (am GMT) on December 6, 2021, and was published in the Federal Register on December 7, 2021. This Order and attestation will no longer be in effect as of am ET [am GMT], June 12, 2022.

You can carry a signed copy with you or fill out one at the airport check-in (they will have the shorter version but you don't need both) . You can just hand it in as you get ready to board the plane, if they haven't requested them before that time.
