I made a mistake assigning my title to the buyer. What should I do? The FL Department of Highway Safety and Motor Vehicles (FLHSMV) requires drivers to apply for a duplicate title by filling out the appropriate form along with other requirements that are mandatory for a smooth procedure. You are allowed to do it either online, in person, or by mail.

If your original title is lost, stolen or mutilated, you must apply for a Duplicate Title. Note: Corrected title applications involving the removal of a name because of divorce or the death of a co-owner or for changing an owner's name because of marriage must be accompanied with a $15 title fee.
