If you are changing your name, date of birth, sex designation (from M to F or F to M), or height, you must visit a ServiceOntario office with your driver's licence and original supporting documents for each change or correction.

The name on your Ontario health care card must be changed in person. Attend any Service Ontario office with your current health card and your original certificate. Take your driver's licence if you want to change the name on your driver's licence at the same time.

DRIVER'S LICENCE The licence number starts with the first letter of the last name e.g., Jane Q. Public's licence number starts with a ?P?. The 9th and 10th number (separated by a dash) are the same as the last two numbers of the person's year of birth. The last two numbers indicate the day of birth.

Driver's licence numbers are unique to the driver and usually last a lifetime.

Name change In order to do so, you will need to visit an Ontario DriveTest Centre with documents proving identification. These documents must show your date of birth, signature and your legal name as it currently is. Your legal name is the one that was registered at birth or changed legally through adoption.

Visit a driver's licence office in person If you attend a service office you can often update your health care card, voter enrolment and driver's licence all at the same time. Take your current licence, provincial health card and original proof of name change with you (eg: marriage / divorce / name change certificate).

In Ontario, if you forget to change your address on your Ontario driver's licence within 6 days of moving to your new address, you may be fined up to $85. While it is not the law to change your address with the various organizations you belong to, you may be fined for not changing your address with them either.
