Nurses must submit a reactivation application through the Pennsylvania license portal at .pals.pa.gov. Once logged-in, you can access the reactivation application by clicking on the pencil icon located beside the license number you need to reactivate.

(1) A graduate registered nurse who wishes to practice professional nursing shall submit an application for a temporary practice permit for a graduate registered nurse on a form provided by the Board and remit the fee specified in § 21.5 (relating to fees).
