It is no longer necessary for affidavits, sworn inventories and attorney's verifications to be sworn to before a Notary Public. The following statute was effective on September 1, 2011: Texas Civil Practices and Remedies Code.

An affiant is a person who is the author of an affidavit, who swears to the truth and accuracy of the statements made in the affidavit. At a minimum, an affidavit of identity must include the following information: The affiant's legal name. The affiant's date of birth. The affiant's current address. The affiant's phone number. The affiant's proof of identity provided to notary public. Oath under penalty of perjury. The affiant's signature. Date of signing.

Personal Details: Provide detailed personal information about the affiant, including their full name, address, contact information, date of birth, and relationship to the person(s) in question. 3. General Affidavits must be signed in front of a notary public. A notary must verify your identification before you sign the document. Make sure to bring ID (like a driver's license or other form of identification) for the notary before you sign the affidavit.

Example: I, Jane Smith, swear that the information in my sworn statement is truthful to the best of my knowledge and understanding. Your statement of truth must be in the first person and you need to identify yourself in it. Keep it short and sweet.

Notarizing an affidavit Once you have filled out the important details on the general affidavit, you must have the document notarized, for which you must appear in person at a notary public in Texas. The notary must verify your identity and see you sign the general affidavit form.
