The title is one double-space below the table number in italicized font. The caption is descriptive, double-spaced, and appears immediately underneath the table or figure. The caption begins with the word Note. in italics, followed by a description, the source information, and the copyright information at the end.

The general advice is to place the figure/table as close to the reference as possible. It doesn't matter - chances are the journal will mess with the formatting anyway. Ideally, figures and tables are on the same page as the reference and (field-dependent?) are put at the top of the page rather than in-line.
