A release of information is a document that gives a consumer the opportunity to decide what material they want released from their medical file, who they want it delivered to, how long the data can be issued, and under what statutes and guidelines it is released.

Some people use the terms ?electronic medical record? and ?electronic health record? (or ?EMR? and ?EHR?) interchangeably. But here at the Office of the National Coordinator for Health Information Technology (ONC), you'll notice we use electronic health record or EHR almost exclusively.

The main difference between EMRs and EHRs is that EHRs are maintained by multiple providers, while EMRs are only maintained by a single provider.

Ing to Florida law, a physician is responsible for maintaining records for at least five years (64B8-10.002). Because malpractice lawsuits can be brought up to seven years after the date of an incident (eight years for some minors), physicians are encouraged to maintain records for the full seven years.

An EMR typically describes a patient's medical information as maintained by an individual clinician or facility. Health information is more holistic, including the condition of one's body, mind, or spirit. An EHR includes information from (and is shared among) many different healthcare providers.

An Electronic Health Record (EHR) is a collection of various medical records that get generated during any clinical encounter or events. With rise of self-care and homecare devices and systems, nowadays meaningful healthcare data get generated 24x7 and also have long-term clinical relevance.
