Soap Note Delete Conditional Fields For Free

The acronym SOAP stands for Subjective, Objective, Assessment, and Plan. Even though therapists don't usually use a traditional SOAP Note format, Catalyst refers to session or progress notes as SOAP Notes.

For HIPAA purposes, a SOAP Note that has been saved or saved as a draft cannot be deleted. A copy of each saved note is stored in the SOAP Notes tab of the Client's record for viewing and editing.

To edit a SOAP Note, navigate to your: Personal Profile. Select SOAP Notes. Type in Search Clients to find and select a Client to open their history. Locate and select the SOAP Note to open the current version. Select Edit to activate the fields and tools. Make changes as needed.

Access SOAP Notes from the Administration Menu in the Administration Section. Locate desired SOAP Note Template and click the Actions ellipse. Select Delete.

Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.
