Today, the SOAP note ? an acronym for Subjective, Objective, Assessment, and Plan ? is the most common method of documentation used by providers to input notes into patients' medical records. They allow providers to record and share information in a universal, systematic and easy-to-read format.

To edit a SOAP Note, navigate to your: Personal Profile. Select SOAP Notes. Type in Search Clients to find and select a Client to open their history. Locate and select the SOAP Note to open the current version. Select Edit to activate the fields and tools. Make changes as needed.

Access the Settings subtab from the Setup Tab in the Students Section. Session Settings will determine if the SOAP Note button should allow a User to create a SOAP Note for the Student in the app. Set Use/Timesheet/SOAP Note to Yes to enable SOAP Notes in the App for this Student.

Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.
