Learning how to write a SOAP note can be one of the most effective ways for clinicians to document and objectively assess, diagnose, and track plans for clients. SOAP stands for Subjective, Objective, Assessment, and Plan. Each letter refers to one of four sections in the document you will create with your notes.

Access SOAP Notes from the Administration Menu in the Administration Section. Locate desired SOAP Note Template and click the Actions ellipse. Select Delete.

SOAP notes include a statement about relevant client behaviors or status (Subjective), observable, quantifiable, and measurable data (Objective), analysis of the information given by the client (Assessment), and an outline of the next course of action (Planning).

For HIPAA purposes, a SOAP Note that has been saved or saved as a draft cannot be deleted. A copy of each saved note is stored in the SOAP Notes tab of the Client's record for viewing and editing.

Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.

To edit a SOAP Note, navigate to your: Personal Profile. Select SOAP Notes. Type in Search Clients to find and select a Client to open their history. Locate and select the SOAP Note to open the current version. Select Edit to activate the fields and tools. Make changes as needed.

However, all SOAP notes should include Subjective, Objective, Assessment, and Plan sections, hence the acronym SOAP.
