Talk to your college's registrar or transcript office about the necessary procedure for disputing something on your transcript. For most universities, there's an official form to fill out, so you can list exactly what you're contesting and the evidence you have to support your claim.

Most universities won't accept an unofficial transcript. There are numerous reasons for this, including that an unofficial transcript may be incomplete. Let them know that there was an error on your transcript, but that it has since been corrected and you would like to provide them with the updated version. Some colleges may be willing to accept the updated transcript even if it's after their deadline, so it's worth a shot.

As a result, the name change reflects on the permanent record of a graduate, including the diploma re-issued. Most institutions of higher learning notarize diplomas without any additional charge.
