Abbreviations are sometimes not understood, misread, or interpreted incorrectly. Their use lengthens the time needed to train healthcare professionals; wastes time tracking down their meaning; sometimes delays the patient's care; and occasionally results in patient harm.

Key components of a medical record: Patient demographic data such as age, sex, nationality, etc. Social screenings such as their profession, etc. Information about their genetics. Medical history and diagnosis received so far. List of medicines. List of vaccinations the patient has received. Lab test results.

This form should include specific details such as the person or organization being authorized, the person or organization being sent the information, the nature of the information being shared, the reason for the disclosure of information, and important statements that the patient needs to understand before they sign.

Abbreviations should only be used if the organization or term appears two or more times in the text. Spell out the full term at its first mention, indicate its abbreviation in parenthesis and use the abbreviation from then on, with the exception of acronyms that would be familiar to most readers, such as MCC and USAID.
