Medicare requires that services be authenticated by the persons responsible for the care of the beneficiary. The treating physician's/non-physician practitioner's (NPP's) signature on a note indicates that the physician/NPP affirms the note adequately documents the care provided.

Medicare requires that services provided/ordered be authenticated by the author. The signature for each entry must be legible and should include the practitioner's first and last name. For clarification purposes, we recommend you include your applicable credentials (e.g., P.A., D.O. or M.D.).

For medical review purposes, Medicare requires that services provided/ordered be authenticated in the medical record. The method used shall be a hand written or an electronic signature. Stamp signatures are not acceptable.

Include the provider's printed name, handwritten signature(s), initials, credentials, license number and National Provider Identifier (NPI). If a provider has various signatures, all versions of the signature should be included on the signature log.

Electronic signatures usually contain date and time stamps and include printed statements, e.g., 'electronically signed by,' or 'verified/reviewed by,' followed by the practitioner's full name and preferably a professional designation.
