An account will be referred to the collection agency for collection if: i. the account holder fails or refuses to comply with the order of the Florida Office for Professional Regulation to provide specified information; ii. a fee is payable to the office or a copy of this letter is received by the agency requesting the referral, on submission of the fee; iii. A copy of the letter requesting the collection is provided; iv. the collection agency has not received any notification of, or received an offer from, the account holder regarding the collection; v. a letter from a collection agency stating that it is acting on behalf of someone who has a Florida Medical Passport or Florida Health Care Identification Card is submitted to the account holder; or vi. a letter of law is submitted to the account holder. The account holder must send a copy of each collection letter to the address listed under “Contact Information” for Florida Office for Professional Regulation.
